Ozempic, GLP-1s Cause Emotional Flattening, Loss of Enthusiasm For Life

A large number of patients taking GLP-1s like Ozempic, Wegovy, Mounjaro, and Zepbound describe a lesser-known side effect that doesn’t appear on the list of side effects, but is logically associated with the mechanisms of the GLP-1 agonist drugs. The official term is anhedonia, also known as emotional flattening or emotional dulling. Some have called it “Ozempic personality.”

This is the symptom where sunsets look ordinary, music loses its appeal, and love for spouses or family members feels muted. The Washington Post reported on marriages with emotional distance, lost spark, and diminished libido.

Dr. Drew described the difference between a “liking” system and a “wanting” system that are fundamentally different. While people who take GLP-1 drugs will still like doing things associated with romance, exercise, music, and socializing, they may lose the drive or the “wanting” to pursue these things.

“The excessive enthusiasm for these drugs is what scares me more than anything,” Dr. Drew said. “I’ve seen very serious medical complications, including death. I’ve seen this apathy develop that can be destructive to people’s lives. They don’t go to work. They’re not interested in their spouses. They’re not interested in their children. The things that usually interest them are just not that interesting anymore. And so they have to be very cautious with how much, how long, and for whom this drug is prescribed.”

Patients on higher doses of tirzepatide and semaglutide note the effect is strongest in the days after injection. Some say they forget to eat and feel worse until they force protein and calories, while others describe a zombie-like state where motivation for anything non-essential vanishes.

GLP-1 receptors are not just expressed in the gut and hypothalamus, but also in the brain reward circuits. That includes the ventral tegmental area and nucleus accumbens, which are pathways that govern dopamine-mediated motivation and the subjective value of rewards. The same signaling that quiets cravings for food, alcohol, and nicotine can dampen the reward value of non-food pleasures. The mind quiets, but life can feel “flatter.”

A 2026 three-patient case series published in Obesity Pillars showed patients had reduced motivation, emotional flatness, and loss of interest in previously enjoyable activities on 15 mg weekly tirzepatide. Symptoms improved with dose reduction or added bupropion.

There is limited rigorous research measuring anhedonia effects from GLP-1 drugs. The STEP program for semaglutide tracked PHQ-9 depression scores and suicidality scales and found no clinically meaningful increase, and even a small reduction in some cases. These trials were funded by Novo Nordisk, the manufacturer of Ozempic and Wegovy, and the authors disclosed fees, grants, and employment ties to the company or competitors. The study also excluded people with baseline psychiatric illness.

These studies were not designed to detect subtle reward blunting. FAERS data have flagged signals for depressed mood and related terms with semaglutide. Dedicated prospective studies using validated anhedonia scales as primary endpoints without pharmaceutical funding are largely non-existent.

A 2024 study of 162,253 case and control obese patients found a “significant association” between GLP-1 agonist treatments and a “195% higher risk of major depression, a 108% increased risk of anxiety, and a 106% elevated risk for suicidal behavior.” The authors noted that an “imbalance in dopaminergic neurotransmission” is “hypothesized to contribute to dysfunctions in the reward system, potentially manifesting as anhedonia and other depressive symptoms.”

Dr. Drew explained that patients on these drugs will still enjoy doing the things, but they lose the drive to do so. One Reddit thread involves patients who have to remind themselves to eat following GLP-1 injections, and that can help offset some of the flat feeling.

On the flipside, voluntary fasting has its own documented health benefits that don’t involve drug-induced halts to natural cravings. Multiple systematic reviews and meta-analyses of intermittent fasting show reductions in depressive symptoms. Clinical observations of therapeutic fasting frequently note early increases in vigilance, well-being, and sometimes mild euphoria. Intermittent fasting produces antidepressant-like effects in stress models, including reduced anhedonia.

Dr. Eric Westman, associate professor of medicine at Duke University, told the Telegraph last year that he has seen the keto diet providing the same benefits as GLP-1s in terms of reducing appetite and “elimination of food noise, without the side effects.”

The article also points to intermittent fasting or time-restricted eating as a good cost-effective alternative to GLP-1s that is safer, more accessible, and longer lasting. A Telegraph article published in April suggests fasting for one day a week is a better alternative to GLP-1s for weight loss, and it is completely free. The author writes that fasting provides improved benefits of reducing inflammation, cholesterol, blood pressure, and risk of type 2 diabetes.

The HighWire has written about ongoing concerns and lawsuits against GLP-1 manufacturers for blindness, suicidal ideation, stomach paralysis, and overdoses. HHS Secretary Robert F. Kennedy Jr. has previously spoken out against GLP-1 drugs as a solution for weight loss, but called it a “tool in the toolkit” to tackle rising rates of obesity in the country that is fueling the chronic disease epidemic.
